Midlothian High School will offer athletic physicals for the 2015-16 school year on Thursday, June 9, from 2:00- 3:30 pm in the Open Commons. The physicals will be administered by the doctors and staff of HCA Virginia Sports Medicine. The cost of the physical is $30.00. Checks should be made payable to Midlothian High School. Please have parts I, II, IV, and V of the physical form completed when you arrive for the physical. Physical forms may be picked up in the main office at Midlothian High School.
